For Referring Providers
Patients referred to PCCO
Patients referred to PCCO will be scheduled for an office visit with a cardiologist. Any cardiac testing that is indicated or requested by the referring provider will be performed the same day as the office visit and the family will receive the results of the testing before they leave our clinic. If additional testing or procedures are indicated, such as a Holter monitor, MRI, stress test, or a surgery or catheterization, that will be coordinated by our clinic after the office visit.
***Please do not use the Randall Children’s Hospital Specialty Referral form. We are affiliated with Legacy/Randall Children’s Hospital but we are not a Legacy Medical Group clinic. All scheduling is done by our dedicated scheduling team and not Legacy’s Centralized Scheduling. ***
✕To refer a patient to our clinic, please follow these steps:
Obtain an insurance referral for office visit/diagnostics from the patient’s insurance company (if applicable).
Fax to our clinic:
- Face sheet/Demographics
- Insurance referral (if needed)
- Pertinent chart notes (documenting reason for referral)
- Results of any previous cardiac testing – Echocardiogram reports, EKG tracings, CXR reports
